Medical Transportation

Transportation buttonTransportation to medical appointments, both in town and out of town, is available for seniors citizens age 60+. Our van travels as far as Worcester and Emerson Hospital. This service is available Tuesday–Friday with pickups starting at 8:00 a.m. The van is returned to Gardner by 4:30 p.m. 

 To find out if you are eligible, please download, complete and print the application. The application may be filled out online or by hand. You can also pick up an application at the CAC. All completed applications must be returned to Gardner CAC, 294 Pleasant Street, Gardner, MA 01440.

Please note that this program books up very quickly, therefore appointments need to be scheduled ahead of time by telephone or in person. For the transportation we ask for a donation. The amount varies depending upon where the appointment is located. However, transportation is never refused based on your ability to make a contribution.

This program is for non-emergency transportation. If you need emergency medical transportation, please call 911 immediately.

*The patient must be ambulatory, as we only have one van.
