Gardner CAC Holiday Toys Application

Residents of Gardner, Ashburnham, Hubbardston, Westminster, and Templeton who meet the income guidelines can apply. The following information is needed and can be emailed to gcac.holidays@gmail.com or dropped off at the office, 294 Pleasant Street, Gardner, MA:

  • Photo ID
  • Proof of residence – phone bill, the electric bill, cable bill, rental/lease agreement, or mortgage statement (any one of those)
  • Proof of household income – SNAP/MA Health/WIC card, pay stubs, SSI/SSDI printout, unemployment, DTA printout (any one of those items)
  • Birth Certificate for children enrolled in the toy program (ages newborn to 12). If you submitted birth certificates previously, they do not need to be resubmitted. If this is a difficulty, please call the office at 978-632-8700.
